This is what was sent to the fall coaches about information needed on the first day of practice.
With report cards, we sent out a green sheet of paper. On the top of the
> paper it says, "Are You Going out for a Fall Sport"? This is their
> insurance information and athletic waiver form. Everyone should hand you
> one of those on the first day of tryouts. They may print one off the
